Caregiver Support

Caregivers often manage medications, appointments, symptoms, transportation, communication, and daily care needs. BloomCare Caregiver Connect supports authorized caregivers so they feel informed, guided, and less alone.

This program is especially helpful for patients with chronic illness, recent discharge, cognitive or functional decline, medication complexity, or high caregiver involvement.

What our RNs do

  • Complete caregiver check-ins when authorized
  • Assess caregiver concerns, barriers, and strain
  • Reinforce provider-approved care-plan instructions
  • Review medication, appointment, and symptom barriers
  • Route safety concerns or care gaps to the provider team
  • Document caregiver communication and escalation activity

Caregiver Connect Details

Helping families stay connected to the care plan

Caregiver Connect is a supportive care coordination layer. It does not replace emergency services, provider-directed care, social work, or licensed behavioral health treatment.

Caregiver Check-Ins

Scheduled outreach to understand caregiver concerns, barriers, stress, and follow-through needs.

Education + Guidance

Supportive education around appointments, medications, red flags, chronic disease goals, and escalation.

Care Team Communication

Helps authorized caregivers communicate barriers and unresolved needs back to the RN/provider team.

Burnout Awareness

RNs identify caregiver strain, confusion, or resource needs that may affect patient safety and adherence.

Post-Discharge Support

Caregivers can receive additional guidance after hospitalization to reduce confusion and missed follow-up.

Documentation Support

BloomCare documents communication, care barriers, education, and escalation steps.
