Healthcare Was Never Meant to Feel This Fragmented
BloomCare™ was created after years of seeing the same care gaps appear across emergency medicine, virtual care, hospital-at-home, remote patient monitoring, care management, precision diagnostics, and clinical operations.
Patients often receive excellent care during appointments, hospitalizations, or procedures. But too often, the support becomes fragmented once they return home. They are left trying to manage medications, understand discharge instructions, coordinate follow-up appointments, monitor symptoms, navigate specialists, communicate with caregivers, and identify warning signs on their own.
For many patients, deterioration does not happen all at once. It happens quietly between visits:
- A patient gains weight for several days before a heart failure exacerbation.
- A patient misses medication refills because of cost or confusion.
- A patient with COPD notices worsening shortness of breath but waits too long to seek help.
- A caregiver becomes overwhelmed trying to manage appointments, prescriptions, and symptoms.
- A recently discharged patient does not fully understand what changed after hospitalization.
By the time these patients reach urgent care, the emergency department, or the hospital, the opportunity for earlier intervention may have already been missed. BloomCare was started to help change that.
The BloomCare Mission

Our Approach: BloomCare exists to fill the space between provider visits with proactive, RN-led, technology-enabled support. We help practices extend their care infrastructure so patients are monitored, educated, supported, and escalated earlier when concerns arise.
Deeply Personal: The need for this level of support became even clearer through personal family experiences with diabetes, cancer, and congestive heart failure. These were conditions that required ongoing education, medication support, symptom awareness, care coordination, caregiver involvement, and timely access to care.
Many of the services BloomCare provides today are the services families often wish they had earlier: someone checking in, someone helping explain next steps, someone watching for deterioration, someone helping coordinate resources, and someone helping patients and caregivers feel less alone.
BloomCare was built for patients who need more support, caregivers who need guidance, and healthcare practices that want to deliver better longitudinal care without building the entire infrastructure alone.

Heena Murray, RN, BSN, founded BloomCare after more than a decade of experience across emergency medicine, virtual care, hospital-at-home programs, AI-enabled remote patient monitoring, precision diagnostics, quality improvement, care management, and digital health operations.
Her career has focused on building scalable clinical workflows, improving patient journeys, supporting distributed care teams, strengthening quality and compliance, and helping healthcare organizations operationalize innovative care models.
Frontline Experience: Working on the front lines in the emergency department, Heena saw firsthand the pain points and care gaps that exist in the healthcare system today. Many patients arrived in crisis after their symptoms had worsened over time. Some needed hospitalization not because their conditions were impossible to manage, but because earlier support, monitoring, education, or care coordination was missing. This shaped her belief that healthcare must move from reactive care to proactive, connected, longitudinal support.
Digital Health Operations: Later, Heena’s work in virtual care, hospital-at-home, AI-enabled remote monitoring, precision diagnostics, and clinical operations showed how nursing, technology, analytics, and structured workflows can be used to support patients more effectively outside traditional care settings. BloomCare brings those lessons together into one care infrastructure model designed for modern healthcare practices.
